11 year old Pom with skin, weight and hair issues

My Pomeranian, Copper, will be 11 this month. She has recently starting losing all of her hair around her tail and all over her back half. Her front half is still full and fluffy. Her hips bones seem to be bonier than before. She is losing weight and itching... any ideas?

Hair loss

Have her tested for slow thyroid. My Pom's hair came back once I gave her thyroid medication that she needed.
