3-year-old pom that ALWAYS pees inside of the house

I have had Buscuit, my 3-year-old pom, since he was only a few months old. We trained him to do his business on a pee pad on an enclosed back deck. Unfortunately, he always seems to pee in our living room (maybe 1 out of 5 or 7 times). There are about three spots that he constantly pees on whenever he decides to.
In order to stop his peeing, we have tried scent neutralizer, punishing him via a good pad as he is doing it, showing him what he has done, and letting him out as often as possible. And while sometimes he goes where he is supposed to, he won't change his habits for anything!
When he does pee, he always runs behind a couch (between the wall and the back of the couch) where he can't be reached, sits and growls. It doesn't matter if he has been caught or not. We have even closed off the areas behind the couches, but he will still pee and get as close as he can to the blocked opening.
Does anyone have any suggestions to help me out...because, MAN do I need them about now!:)
