When my pom experienced this it was due to gallstones. And we had to get surgery to get the stones removed.

could be Colitis by: Michelle from SFO
My teacup Pom started having jelly/mucous/bloody poop when she was about 1 yr. old and my vet sent me to a specialist. This condition was diagnosed as COLITIS - inflammation of the intestines due to food allergy. Instead of a colonoscopy, which is not recommended for a teacup Pom, we elected to try a prescription diet - ZD Ultra Allergen (Hills) - and it worked! Thank goodness. She has been doing well for the last 3 years on this diet and you just have to be very careful to not let your dog eat anything else, as any food will trigger the jelly stool occasionally, when she scavengers bits of food dropped on the floor accidentally. And you will hear her belly making gassy & weird little sounds when the colitis is acting up, but consistency with the restrictive diet is the solution. Good luck!

fecal workup by: Anonymous
Mine is doing the same thing. Vet said she has a bacterial imbalance. It could also be intestinal worms or parasites. Your pom can get this very easily. Vet needs to do a fecal work-up.
