Randomly pees or poops right next to the wee pad

My 6 month old Pomeranian has been going consistently on a wee pad since she came home from the breeder at 10 weeks.

Very recently she has started going right next to the wee pad on the kitchen floor. Sometimes she'll pee on the pad then get off and poop on the floor. The behavior is very random as sometimes she'll still go consistently on the wee pad.

Nothing's changed in her routine so we can't figure out what's causing this change in what seemed to have become her routine. (She has been crate trained and has never had an accident in the crate)

Any advice/suggestions you can offer will be greatly appreciated.
